This form will allow Lamoni Campus Graceland University students to contact the Proctoring Service Coordinator to request a reservation for test proctoring. This request should be made at least 72 business hours (3 days) before the scheduled exam time. 

Completing this form does not mean that a reservation has been made. Please check your Graceland email for an email from proctoringlam@graceland.edu to confirm that your reservation has been made.
