This form will allow Lamoni Campus Graceland University students to contact the Proctoring Service Coordinator to request a reservation for test proctoring. This request should be made at least 72 business hours (3 days) before the scheduled exam time.

Completing this form does not mean that a reservation has been made. Please check your Graceland email for an email from proctoringlam@graceland.edu to confirm that your reservation has been made.

Faculty Form: Test Information for Proctor

INDEPENDENCE AND ONLINE (WWW) CAMPUS FACULTY: email proctoringindp@graceland.edu for services.

LAMONI CAMPUS FACULTY: The link to the form below will allow Lamoni Graceland University faculty to upload an exam or quiz to the Proctoring Service Coordinator.

Please make sure you are logged in to fill out this request form and upload the exam no later than one day prior to the exam date. If a professor does not wish to upload the exam they may deliver the exam to the Student Disability Services Office, Newcom Student Union Room 31. Please provide in sealed envelope with your name and the student's name on the envelope. You may slide it under the door if no one is there.

In the event that there is not a test proctor available, the Proctoring Coordinator will work with the faculty member to provide testing alternatives.

Test proctoring coordinated by the Proctoring Coordinator and is not mandatory. Faculty members may proctor or arrange proctoring for any and all exams for their students as long as the Letter of Accommodation is followed.
