The protocol is for a student to call or email teachers to inform them of the reason for an absence from class. 

In addition, if they want the absence to be considered as an excused absence they will need to fill out the Absence Request Form:

* This request form will ask for supporting documentation for the student's requested absence. 

All medical records are maintained in strict confidence. No information is released without the student's written and signed consent.
