Provided by the Community Health Centers of Southern Iowa

Graceland University partners with Community Health Centers of Southern Iowa (CHCSI) to service the health needs of our students.

CHCSI is located within walking distance from campus at 802 E Ackerly Street, Lamoni, Iowa 50140. Students may make an appointment to be seen by calling 641-784-3371.

This clinic operates just like any other health clinic in regard to office visit charges, insurance billing, etc.

Health Requirements

Graceland students are required to complete the Health History and Immunization Form. The form only needs to be submitted once for the entire time of enrollment.

IMMUNIZATIONS:

Required: MMR #1 and MMR #2 (Mumps, Measles, and Rubella).

Recommended:The Hepatitis B series, Meningitis and HPV(Gardisil).

Health Insurance

Graceland students are REQUIRED to have an insurance plan that is valid in the state of Iowa. Please note: Many policies, including Medicaid are only valid in the home-state.

USA residents that need to purchase insurance may choose to use this resource: www.getinsured.com/lewer or call 866.602.8466.

International students will be automatically enrolled with LewerMark Health Insurance which will be charged to their student account each semester. Here is a short introductory video regarding the insurance plan in the USA.

Current Student Information

Absences

The protocol is for a student to call or email teachers to inform them of the reason for an absence from class when ill.

Documentation of an office visit is available to the student upon request.

Confidentiality

All medical records are maintained in strict confidence. No information is released without the student's written and signed consent.

Immunizations

Immunizations that are required and recommended are to be documented in the Health Form that can be accessed from this page and submitted electronically.
